Last this was discussed, the .pdf which such a tool makes was supposed to be wrapped up in some .rtf code which would contain the baseline adjustment.
I see a checkbox in the Service Preferences for ``Align the equation in the original text'' which notes that ``This may prevent LinkBack from working'', but a quick test w/ a nonsense equation in InDesign has it not working even though it's checked.
Upgrading to 2.4.1 didn't change this.
Not sure what happened --- I didn't track this after the initial discussion.
