I don't know how to change the prelude in order to accommodate your description. Although I did not notice what you describe, prior to reading your message, I prefer the existing setup.

With respect to your problem (2), you can place
\usepackage{cleardoublepage} % inserts blank page if chapter ends on odd page % applies to front matter also
in your prelude. In consequence, if the last page in a chapter has neither text nor graphics and is even numbered, then it will appear as a blank page. This will apply also in your 'Contents' pages, List of Tables, etc. I don't use the package, but it may be to your liking. :-)
