The reason for this is that threading is based on something in the
email message's header information, which is normally not shown
in your email client --- though there should be a way to see it.

For example, in the message that this is a reply to, 
there is are header lines:
    
  From: Herbert Schulz <herbs at wideopenwest.com>
  In-Reply-To: <56DB6D56-7A67-4313-9210-CD68D48F84F7 at gmail.com>
  Date: Thu, 10 Feb 2011 16:00:37 -0600
  Message-ID: <08AF0A5E-9D18-419D-91BD-1E62A79BD4D8 at wideopenwest.com>


and Alain's message before that has:

The linking is achieved by the common identity string:

  <56DB6D56-7A67-4313-9210-CD68D48F84F7 at gmail.com>

in the ID of one, and the Reply-To of the next.
Thread history is maintained by the other "References",
but it is not mandatory for email clients to always use 
or supply this extra information.



So you can see that "etiquette" is just a simplified way
of stating what is needed to properly maintain the
usefulness of this complicated structure, the details 
of which you probably do not wish to know.
