[OS X TeX] Need Help With Indexes

Folks, I am writing a biography for my mother and am having problems getting LaTeX (or MakeIndex) to create the sorted index (.ind file) from what looks like a legitimate .idx file. I have attached my source file, log file and index file for your review.  Any insights would be appreciated.  Thanks.  Sam.....
