Blame LaTeX consultants who back when they could, would charge $1,000  
for 5 minutes of work.

The problem for publishers is that they can put out an ad for people  
to do work in Quark or InDesign and get a stack of résumés --- there  
are far fewer people w/ (La)TeX skills and most of them have other  
interests than toiling away as galley slaves:
