Howdy,

The fact that the system can't find kpsewhich (the return with no  
output --- the output should be /usr/texbin/kpsewhich) is a bad sign!  
You may have to re-install MacTeX. I'd stay away from tlmgr for a  
while; it needs some updating to get more stable.
