[OS X TeX] What does it mean?

Hi All

Got an interesting error message that I have not seen before.

I'm using plain tex on OS10.3.9

The error is;

! pdfTeX warning (ext4): destination with the same identifier 
(name{page.1}) ha
s been already used, duplicate ignored
<to be read again>
                    \penalty

This is repeated six times for for pages 1 - 6.

Any clues would be gratefully received.
