First, [Update Self from Remote]: this will update the local copy of  
the i-Package.

Then, [Install & Configure]: this will install the updated copy, and  
afterwards configure the install (probably offering you a few choices).

[Configure] runs the configure script alone. It's only interesting  
for changing the configuration of an already installed i-Package.

The doc of i-Installer, accessible from the Help menu of i-Installer,  
tells much more about these topics. See, in your case, the help page  
"The i-Package Window".
