Yes to all three. One can force mpm to work with /Library/TeX/ 
Distributions/.DefaultTeX/Contents/TexmfLocal/ by hardwiring it at  
the build's configuration stage with:

./configure MIKTEX_INSTALLROOT=/Library/TeX/Distributions/.DefaultTeX/ 
Contents/TexmfLocal/

Otherwise, it will hardwire the location found by a call to kpsewhich  
at the same stage. I gather that if we were enough people to nicely  
ask MikTeX's developper, he would include the TeXDist mechanism into  
mpm's build...
