[OS X TeX] Very (Mac)-specific mpm problem

Probably it is not the source of the problem but you need to run mpm as root:

% sudo mpm-update-db

and usually mpm instantly starts with
WAITCONNECT => PROTOCONNECT

What version do you have?

%mpm --version
MiKTeX Package Manager 2.5.2392 (MiKTeX Tools 2.5 Beta 14)
Copyright (C) 2005-2006 Christian Schenk

Have you compiled it or copied from somewhere?
