[OS X TeX] problem with figure numbering

hi all,

i'm writing a thesis and my figure numberings seem pretty random.

I have  different chapters and it looks like the figure numbers start 
counting at 1 again at the start of each chapter.

also, sometimes, doing a \ref to the figure turns up the right number 
(for example \ref give 2 if the figure number is 2). at other times, i 
get a 2.4.6  from \ref  for all the figures if i'm in i chapter 2, 
section 4 subsection 6 for example.

any ideas on how to correct this?
