[OS X TeX] Small italic caps & pxfonts?

In my previous post I forgot to add that you ought to use a font which
has italicised small caps, of course.  In XeLaTeX (with the fontspec
package) you can use the fonts available in your mac and some (e.g.,
Hoefler Text) have such shapes.
