In the "Macro" menu, select "Macro Editor"

in the resulting window, select the macro that you want to assign a 
keyboard equivalent to.
The macro's text will appear in the right panel.  At the bottom of the 
window is a box
where you can type the keyboard equivalent "Key: ", and select any 
additional modifiers
(e.g., option/shift/control)

Not sure on your other questions...
