Given an ordinary font family, it is in principle possible to create
slanted small caps and bold small caps as transformations of regular
fonts. You can do this with Fontinst. The official documentation is
on TeX Live: texmf-dist/doc/fontinst/base/manual/fontinst.pdf.

There is also a Font Installation Guide by Philipp Lehman on CTAN
and therefore on the texlive dvd, at
info/Type1fonts/fontinstallationguide/.

For slanted small caps there is an additional twist: `slanted' and
`small caps' are both font shapes, in LateX terminology. The
combination `slanted small caps' would be a new, non-standard shape
for which you would have to pick a name. So you would need to
understand the low-level LaTeX font interface, and understand .fd
files.
