[OS X TeX] PSU Thesis

Dr. Gray,

I downloaded, and looked over your PSU thesis .cls file.  I changed a 
few minor things in the .cls file itself such as the school name, since 
I'm not a PSU student.  I then went ahead and entered my name, thesis 
title, graduation date, thesis advisor name, reader names, etc. of the 
"Your Name"-Dissertation.tex file.  I then tried to have teTex process 
the LaTeX file using TeXShop, and got the following errors:

I know where the eqlist.sty file is, but I'm not sure I should move it 
>from it's original place.  I haven't really begun entering anything into 
the Appendix, Chapter 1, Chapter 2, etc.  .tex files, as I just wanted 
to see if things would work out for the title and signature pages first 
before moving on.  Do you recognize what the TeX Log is spitting out 
here?  Is it just a matter in which i need to begin entering the rest 
(e.g. Appendix, Chapter 1, Chapter 2, etc.) for this .cls file to work?
