[latexrefman] Move \makeatletter near to \newcommand

I moved \makeatletter & \makeatother in with \newcommand and \providecommand.  I put \@ifstar in there also.  I also promoted Environments to a section as it was alone at that point.

I'm not sure what to do with CTAN.  Seems wierd to have it be the very first thing.  I wonder if it belongs as its own chapter, if we ever do a section on amsmath, etc.

I don't trust my ability to get the adding and deleting of the split .html files right by hand.  So there needs to be a target, it seems to me, that gets done just before the commit.  But it is giving me an error that I don't immediately see how to fix and I need to go.  I made the --force deletions by hand and committed what I had, and commented out that target for another day.
