    1) I'm not sure where to put MakeUppercase.  A new section for
    middle-of-writing commands?  Ideas?

The most expedient place that comes to mind is a new section in the
"Special insertions" chapter.

OTOH, if there are lots more such middle-of-paragraph commands to be
dealt with (?), a chapter on "Text transformations" (or some such) could
be appropriate ... --thanks, k.
