  Start new pdftex.def based on dvips.def

This is a first pass: simply taking code from existing pdftex.def
and adding to dvips.def where the two define the same commands.
Currently graphics inclusion is not covered as the functions are
not strictly equivalent (eps vs pdf/png/jpb, etc.).

A few things might want to move from here to graphics, notably the
scale normalisation. However, unlike the additional keyvals (to be
covered later), this is not user-facing code so the issue is less
pressing.

A guiding principle here is to stick to what is available in plain
+ miniltx. In particular, the plan is to avoid needing the HO stack
to get things to work.
