[l2h] Inline math symbol with no descender

Thanks for responding so quickly.  I did some experiments last night to
make sure it was easily reproducible.  The problem arises when both
MATH_SCALE_FACTOR and EXTRA_IMAGE_SCALE are larger than 1.  In the example
below, I've set them both to 2, but other values give similar results.

The example is at
