The problem is that you need a later texmf tree, and you need the current
pdftex.def (the one on www.tug.org/applications/pdftex, not the one in the
graphics package).  The first problem is fixed in the current teTeX tree
(has been for some time), and the second is also an out-dated version
problem.  I do suggest you update your texmf tree carefully now.

Nope, it works fine on extensive tests for me.  It seems to me that your
installation has some bugs, though!
