The pathological fonts are only half of the story: the flood of useless
kerns introduced between accented characters the other half. When these
are dropped (like duplicated kerns) there is much less strain for all
the components involved. Another step towards efficiency would be SKIP
instructions but that requires additional non-trivial structure in the
program.
For the practical case it remains always to simply increase the minimal
kern limit - especially for pathological fonts ;-)
