Chris,

associated file are not external. They are embedded in the pdf and
"associated" to a (structure) element.

mathml is imho in the long run the best format to navigate through
the math (if the screen report it), and adding it as file will be
easier (and so more probably done) than tagging the math element
one-by-one.

The main question here is if and how the screen reader and the other
tool do/will support this. Definite and clear answers are quite hard
to come by.
