This is what I am working on, and have achieved with several significant examples, chock full of mathematical content. These will be part of my TUG presentation in Rio, later this year.
They are not yet available online.

Examples of PDF/UA documents built using LaTeX
There are some examples where the mathematics is treated like ordinary text.
Those PDFs validate for the stated standards, but should not be considered properly Accessible,
as the mathematical content is not marked as such.
I produced these, to show that it is possible to validate against standards, without actually
conforming with the intent of the standard: "Letter of the law" vs "intent of the law".
I suspect that some publishers might contemplate doing this kind of thing, which really
would be unfortunate. Some may be doing it already.
Look in particular at the Annals of Mathematics, example document.
This is roughly 30 pages, with all kinds of mathematical structures.
I’d really like to have comments about how this sounds, using a screen-reader,
or other Assistive Technology.
