[texhax] hyperref JavaScript Examples?

TeX Fans,

Does anyone have any hyperref JavaScript examples?  Specifically I'd like to
add an "input mask" to some date and time fields so that the separator
characters are filled in automatically.  For example, a user typing 02262019
in a date field would result in 02/26/2019.
