The solution did work for me. <br><br>Briefly, the problem is attributed to bidi's
restoring the <code>book</code> definition of the <span style="font-family:monospace,monospace"><code>theindex</code></span> environment for no apparent reason.
The solution is to insert the following lines between the <span style="font-family:monospace,monospace">\usepackage{imakeindex}</span> and <span style="font-family:monospace,monospace">\makeindex[intoc]</span> lines:<br><br>
