[XeTeX] Combining pair of vowels to single base char in Hebrew

I can reproduce this issue with the latest XeTeX release, but not with
the development version linked with the latest HarfBuzz, so I think it
was a bug in the HarfBuzz version used in TeX Live 2013.

There is no immediate fix, unfortunately (apart from building XeTeX
yourself, or using a Linux distro packages that uses an up to date
HarfBuzz).
