[XeTeX] combining characters in isolation

Thought about the glyph ID route, but unfortunately this is all about 
portability.  The files we're creating are actually in DocBook XML, and 
get converted to Xelatex by dblatex.  So I'm afraid the glyph ID idea 
won't work...
