Gosh I hate to reply to my own comments...

Dick Koch said he had no problem with xelatex and using xdv2pdf with the Hoefler Text font; i.e., with the file I sent to the list. After trying it again I also had no problem. I'm still having a problem using a custom format based on plain, xdv2pdf and the Hoefler Text font; I get a completely blank page with a warning that the pdf may be wrong.
