Good question. I think that you might get some answers at comp.text.tex
where hyperref is discussed fairly frequently. I looked fairly  
extensively
and could find no hints about the solution to, or cause of the problem  
that
you are having -- so I don't think that you would be running the risk of
asking a FAQ.

I'll be interested to learn what you discover about this.
