OK thanks - I'll give it a go (I do usually use the command line!).  Any 
modifications to the tree have been made in response to suggestions or 
instructions on this list - but I think I have a list of them.  If I recall 
correctly, I had to make some in order to get PSTricks to work at all.

It's not linux-like commands, it's just commands :-)

They will. TeX Live works (allmost) the same on windows as on linux.

But if you don't feel like typing commands in a command line, you can do
the same using a graphical interface:

1. run the TeX Live manager (tlmgr) from
Start Menu -> All Programs -> texlive

2. Click the "change" button on top (besides the "load" button) then
select the "default net location".

3. Move to the "update" tab. In the list on the left, find and select

bin-texlive
texlive.infra

and click the "update selected" button.

4. Then (since your are on windows) you'll see a message box asking you
to run the final step by hand: do it using the shortcut created on your
desktop.

5. Now re-run tlmgr, click the "load" button, move to the "update" tab
and click "update all". This may take a while, but afterwards you have a
fully up-to-date TeX Live with needing to update anything manually from
CTAN.

That's why you modifications should be done in texmf-local and not in
the main texmf tree.
