  On the Mac with an “Extended U.S. Keyboard”, Alt-[ produces “, and
Alt-Shift-[ produces ”.  The simple quotes are obtained with Alt-] and
Alt-Shift-], respectively.  I'm on Tiger but I don't expect things have
changed much.
