Yes, it is. With the Perl script which Peter Heslin posted you can  
convert GRAhyph4.tex:

	http://www.ctan.org/tex-archive/language/hyphenation/elhyphen/ 
GRAhyph4.tex

into Unicode. You must then edit language.dat, adding a new entry or  
writing the name of the new hyphenation file instead of grhyph.tex,  
and recompile XeLaTeX format to load hyphenation patterns.
