Oops - I changed the entry in the index, but missed it before the \index.

Anyway, thanks for the comments and the quote-image for the title page.
I've implemented them in my version and am currently working on
implementing on some other suggestions sent to me off-list. Once that's
finished, I'll post new drafts.
