Well, it does get extended, hence XeTeX's .xdv format and xdvipdfmx

That said, the OP does have a point --- it is faster to write out .dvi (or some extended variant) and then generate a viewable version of each page on an as-needed basis --- but on the gripping hand, as fast as computers are these days, the ability of a user to perceive this speed differential has diminished to the point of imperceptibility.
