OK, tagged pdf will not solve everything, but it's currently the only
"working" technology (and you asked for the screen reader and tagged
pdf). Math is sorely lacking there, and is also not really missed by
e.g. the PDF/UA group. But also lacking is any participation by the
TeX community, e.g. AMS.

AFAIK markup of math is hard; do we really want to go the MathML way?

Btw: I'm wondering why nobody mentioned the TeX-math reader presented
more than 10 years ago at a TeX conference. I forgot the name of the
author. :-(
