I believe \printindex gets redefined after \backmatter or \appendix.

This sort of thing is controlled by the current pagestyle. The default 
for chapter openings is plain, so you need to switch it to be the same 
as the other pages, using something like \thispagestyle{headings}, but 
hard to be sure w/o knowing what documentclass you're using. Have you 
considered memoir? It's flexible, easily changed, and well documented.
