[texhax] Generating index

Hi, all.

I saw that also TeX for the Impatient uses an external program to generate
the index,
and even T. Winograd and B. Paxton in TUGboat 1, 1980.
Is that always necessary, or maybe it could be possible to create an
automatical index only by TeX macros?
If so, are there any examples?
