Given below are the commands you need to execute from your shell prompt:

1. htlatex scr

2. tex '\def\filename{{scr}{idx}{4dx}{ind}} \input idxmake.4ht'

3. makeindex -o scr.ind scr.4dx

4. htlatex.scr
