[tex-eplain] indexing appendices

    The documentation for makeindex should include ind.pdf (or .dvi or
    .tex) which covers page_compositor and other makeindex style file

It is on ctan, http://ctan.org/indexing/makeindex/paper.  No license is
ever stated (not surprisingly, since it's written as a journal paper
rather than documentation, and things weren't so cut and dried circa
1990 anyway), so we can't distribute it in TL.

I guess I'll try to write Harrison and Chen about it.  Sigh.
