[pdftex] Win32 pdftex with Tom Kacvinsky patch

thank You for
ftp://ftp.dante.de/pub/fptex/standalone/pdftex-14h-patched-20010618-win32.zip

As a user of MiKTex, in the appropriate directories I simply replaced pdftex.exe and pdftex.pool by Your versions. This does not solve the problems. 
I know, this is the wrong place. But does anybody know, what additionally must be done in MiKTeX?
