Run these two commands in the terminal, reboot in safe mode and then reboot normally. This worked reliably for me in Yosemite. 

/System/Library/Frameworks/CoreSe\
rvices.framework/Versions/A/Framew\
orks/LaunchServices.framework/Versi\
ons/A/Support/lsregister -kill -seed

sudo find /var/folders/ -name com.apple.dock.iconcache -exec rm {} \;
