Howdy,

Have you checked the Source file to make sure that something didn't actually get duplicated in the text?

Are you using TeX Live 2015 as installed by MacTeX-2015 on both systems? Have you updated both systems? 

PS: the LaTeX macros include fixltx2e under TeX Live 2015 (TL2015) so explicitely suing fixltx2e shouldn't be needed but I don't think that should be a problem since fixltx2e.sty is just a stub in TL2015.
