I have had some of my new code "mysteriously" not appear in a pdf  
file; even though I am embarrassed to say this, when I've look through  
my file it has been because I somehow managed to type the code after  
the "\end{document}" command.
This is not to imply that you've done such a thing, but to point out  
it can be something extremely simple.
