[OS X TeX] Nested \textbf{\textsc{xyz}}

Using computer modern you can get a reasonable looking bold small cap  
with usepackage{bold-extra}.
\textsc{\textbf{xyz}} should produced what your coworker is seeking,  
unless there is a need to really magnify the letters.
