Howdy,

If you want to use the GUI interface for tlmgr you must install the  
Tk-804.028.pkg found in a subdirectory of /Applications/TeX/ (sorry I  
can't be more specific since I re-names that folder). Also, note that  
you must have X11 installed; not a problem with OS X 10.5 since it is  
automatically installed but not automatically with OS X 10.4). Note  
that the GUI is not the pretty one you might be used to.
