Just now I had time to try this modification.  I have no compilation
errors.  Just many of these warnings:

LaTeX Warning: Label `McArthur:1990aa' multiply defined.


I get the citations in the text correctly.  However, in the references
I have a comma when I should have an &.  That is the only problem that
I can see.

BTW, I have kept the modification to hyperref.sty that you suggested.
