[OS X TeX] TeX4ht not installed in TeXLive?

I would suggest to get it from the mouth of the horse
http://www.cse.ohio-state.edu/~gurari/TeX4ht/bugfixes.html
(fonts including) rather than from CTAN.

Note that you need to install a lot of unix and mac binaries too
