Both are helpful. The first edition of the companion contains a  
chapter on bst hacking, including some examples. I would assume they  
kept that in the seconds edition.

I always feel I need to switch my mind into reverse gear to read bst  
files. Seriously: start reading from the end of the file and it may  
make sense after all.

Small changes are not that hard, but deeper changes are far from  
trivial. It pays off to have a starting point close to what you need.  
'tex makebst' is very helpful in this respect.
