It's interesting that much of what people want is already available 
w/in TeX itself (customized formats), using add-ons like metapost, or 
alternative front-ends such as emacs/auctex w/ instant preview and 
dvichop.

I think the biggest improvement which is needed for TeX per se is 
better hooks between the typesetting engine and the output routine (so 
as to improve on page breaking) --- probably such effort can be rolled 
into making things like dvichop more widely available.
