[OS X TeX] Problem with fancyhdr on OSX

Doubtless there are different versions in the MikTeX distribution
and what you have on the Mac...

which is surely quite old now.

The command:

    grep offset `kpsewhich fancyhdr.sty`

returns nothing, so certainly \fancyhfoffset and \fancyheadoffset
are not defined in this version of the package.

for a later version of the package.
